I have encountered too many to count, but here are some honorable mentions:
Playing Apex Legends, squad wiping when suddenly a Valkyrie with an L-STAR lights me up for 225 damage, I obviously, died.
Playing random Roblox games with friends, we found a game where you have to dodge obstacles, my friend got hit by a flying dumpster and his game crashed.
Playing Team Fortress 2, I was walking around as Spy in cloak, I walked over to enemy spawn and saw all of them Kazotsky kicking, I uncloaked and joined them, we kicked into the enemies sentries, and I got blasted into the shadow realm.
